Know your results: Track your procedures and labs

As you undergo procedures and labs, keep records of them including results from each. If pictures are taken at an endoscopy, ask for a copy. If you have a blood test, get a printout of your results.

These hard copies, along with details recorded in your  procedures and labs record (pdf), will give your adult physician an excellent idea of the patterns or history of your IBD. Use the form to improve upon past experiences; if you have a particularly bad or good experience one time, try to figure out why, so you can change or repeat that factor the next time you have the same procedure.

Use the procedures and labs record form (pdf) to record this information so you can share it with your doctor. Be sure to note the type of procedure, date, location, reason for it and the results. Include any sedatives prescribed as well as their effects.
