Rotation Site Description
The Texas Children’s Cancer Center and Hematology Service is the largest of its kind in the United States, serving patients with all types of oncologic and hematologic disorders. Nearly 25,000 patients will be seen in the outpatient clinic, with approximately 1600 of those patients presenting with new diagnoses this year. Patients and family members come from 35 states and 26 countries to Texas Children’s for treatment. The primary site for this rotation is the 9th floor of the West Tower of Texas Children’s Hospital, an open ward comprised of 36 inpatient beds.

Rotation Responsibilities
Provide clinical pharmacy services to pediatric hematology and oncology patients, including:

  • Patient profile review with problem medication order identification/resolution.
  • Understanding the pharmacology, dosing, indication, and anticipated adverse events (and the management thereof) for each medication used.
  • Rounding and interacting effectively with the multidisciplinary Hematology/Oncology teams.
  • Participating in various learning opportunities as appropriate, including Attending/Fellow lectures, Drug of the Day, Pharmacotherapy Nursing Rounds, Pharmacotherapy Rounds, Feigin Rounds, Tumor Board, and Grand Rounds.
  • Providing pharmacokinetic, nutrition, and electrolyte recommendations as indicated on each patient.
  • Providing timely and accurate answers to drug information questions.
  • Monitoring for and reporting of adverse drug events as appropriate.
  • departmental initiatives as indicated.

Activities
Rounds Patients are separated into two teams, read and blue. The post-call team rounds first, followed by the on call team. Rounding times may vary; therefore the resident is expected to be present during the following times for rounds (slight changes may be made on a daily basis):
        Monday: 8:30 am – 12:00 pm
        Tuesday: 8:15 am – 10:30 am (may resume from 11:30 am – 12:00 pm)
        Wednesday: 8:30 am – 12:00 pm
        Thursday: 9:15 am – 12:00 pm
        Friday: 9:30 am – 12:00 pm

Required Meetings:

Meetings as scheduled with preceptor.

Attending/Fellow lectures:

Mondays and Fridays at 12 noon, 9WT work room

Tumor Board: Tuesdays at 4 pm, TCH Auditorium

Feigin Rounds: Tuesdays, 10:30 am, 6th floor Neo conference room

Grand Rounds: Fridays, 8:30 am, TCH auditorium

Hematology/Oncology Edu: one Tuesday per month, 3:00 pm, Cancer Center Clinic Pharmacy

Nursing Pharmacotherapy Rounds: 2nd Thursday of the month, 2:00 pm, 9WT charge room

Pharmacotherapy Rounds: 3rd Thursday of the month, 3:00 pm, TBA

    • Acute lymphoblastic leukemia, Acute myelogenous leukemia, Chronic leukemias
    • Burkitt, Hodgkin, and non-Hodgkin Lymphoma
  • Solid Tumors
    • Osteosarcoma, Rhabdomyosarcoma, Ewing sarcoma family of tumors, Retinoblastoma, Neuroblastoma, Medulloblastoma, CNS tumors
  • Supportive Care
    • Chemotherapy-induced nausea and vomiting, acute and long-term toxicities of chemotherapy, pain management, nutritional/electrolyte supplementation
  • Hematology
    • Autoimmune hemolytic anemia, Aplastic anemia, Hemophilia, Sickle Cell Disease, Idiopathic thrombocytopenic purpura
