Ambulatory Care: Heart Transplant Clinic Rotation

Rotation Site Description
The heart transplant clinic sees pediatric patients that have undergone cardiac transplant or who are being evaluated for cardiac transplant. A multidisciplinary approach is utilized in the care of these patients. Pediatric cardiologists specializing in heart transplantation and heart failure are the primary care givers, however, pediatric electrophysiologists, dietitians, social workers, nurse managers, and pharmacists are actively involved in patient care.

Rotation Responsibilities
Provide clinical pharmacy services to pediatric cardiac transplant patients, which includes:

  • Understanding the basic pathophysiology of cardiac transplantation and associated morbidities.
  • Knowing the pharmacology of each drug used
  • Knowing the indication for each drug used.
  • Ensuring appropriate dosing for each drug used based on height, weight, and renal function.
  • Counseling of patients regarding prescription and non-prescription medication regimens.
  • Counseling of patients regarding drug/drug or food/drug interaction.
  • Pharmacokinetic consultation on any. medications monitored by serum levels.
  • Reporting of adverse drug events and medication incident reports.
  • Documentation of medication regimens.
  • Interact effectively with members of the multidisciplinary heart transplant team.
Activities
Heart Transplant Clinic: 0900 – 1200 Thursdays on the 20th floor cardiology clinic.

Required Meetings:
Meetings as scheduled with preceptor.

The resident will be expected to complete a small project or in-service during the clinic period.

Topics to be discussed
Pharmacotherapy of Pediatric Heart Transplantation:
  • Immunosuppression: Cyclosporine, prednisone, mycophenolate, tacrolimus, azathioprine
  • Heart Failure: ACE inhibitors, Diuretics, beta-blockers, digoxin
Pharmacotherapy of Co-morbidities:
  • Transplant diabetes
  • Transplant coronary arterial disease
  • Chronic Renal Insufficiency
  • Narcotic Weaning Regimens
  • Cardiac Arrhythmias
