Neonatal Intensive Care/NICU Rotation

Rotation Site Description
The Newborn Center at Texas Children’s Hospital is the largest neonatal intensive care unit in the country and is comprised of both a Level 3 Nursery a Level 2 Nursery. The focus of this rotation is to expose the resident to the provision of complete pharmaceutical care services to the NICU patient population. The resident will work closely with medical residents and neonatal nurse practitioners while being exposed to pre-term infants born as early as 23 weeks of gestation, as well as term newborns with congenital anomalies.

Rotation Responsibilities

  • Daily participation in the NICU multidisciplinary team-based rounds
  • Daily patient profile review with identification and resolution of patient medication problems
  • Learn the basics of neonatal pharmacotherapy
  • Develop an understanding of the management of critical patients in the NICU
    • Drug dosing and fluid/electrolyte management in neonates
    • Post-operative care: sedation and pain management
    • Nutritional management of the critically ill infant and management of TPN cholestasis
    • Sepsis and meningitis
    • Congenital heart defects
    • Neonatal seizures
    • Respiratory disorders: apnea of prematurity, respiratory distress syndrome, pneumothorax, bronchopulmonary dysplasia and pulmonary hypertension
    • Neonatal necrotizing enterocolitis
    • Neuromuscular blockade
    • Neonatal abstinence syndrome
    • Hypotension of the neonate
    • Metabolic derangements
  • Provide pharmacokinetic consultations
  • Provide drug information services
  • Participate in neonatal codes, having knowledge of Neonatal Resuscitation Program (NRP)
  • Determine if immunizations are up-to-date
  • Understand the medications used during Extracorporeal Membrane Oxygenation (ECMO)
Activities
Rounds:
0830 – 1200(may run later depending on attending), Monday – Friday.

Required Meetings:
Meetings as scheduled with preceptor.
Physiology Conference, Wednesdays at noon (6th floor Neonatology conference room)
M&M Conference, Fridays at noon (6th floor Neonatology conference room)
Journal Club/ECMO Conference Mondays at noon (6th floor Neonatology conference room)
Grand Rounds (as topics are appropriate)
Hand Off Communication Daily at 4pm PICU Satellite Pharmacy
Other meetings deemed appropriate by preceptor
